Reporting a Bias
PLAN takes discrimination and harassment complaints seriously and has integrated policies prohibiting those behaviors throughout our employee manual. Any staff member subjected to or witnessing these types of situations must report them promptly to the department of HR/DEIB.

This form is also used when reporting bias or discrimination, the appropriate category must be checked for each submission.
